Relinking problem with 10.4

Folks I keep having relinking problems with 10.4. A clip will be offline, and I'll attempt to relink to the source file from which it came. I'll get relink errors that there's not enough media to cover the duration or channels must be the same. Yet the offline media and source have the same duration, channels are all there.

I don't know what's happening with 10.4, but I did not have this many issues relinking before. Any ideas. Hopefully it's just human error, but I can't seem to nail down what's causing this to happen.
