Used media smart collection and file name issue?

I have a smart collection that is "Used Media" is "Used" that I use to create proxy files for clips that are used in my timeline. I just noticed after creating proxy files for all these in this collection that some in my timeline are not being generated. Further investigating i've found that these clips in the timeline also do not appear in the Used Media smart collection. It appears this is because of the clips having the same Finder file name as other clips in the library. They show up in my browser as "Clip name 1" but show up in the timeline as "Clip name".

Any thoughts on this?

Screen shot? When I do that, I see each range of a master clip used in a timeline as an individual clip with the same name as the master (original) clip.
