big issue importing clips with P2 card

Hi everyone
we work in the news and almost ten editors have troubles over the months with importing clips from P2 cards , working on dvcpro HD.
when lauching the import and ingest the clips onto Data, we have several issues.
- it forgets some clips, even when if we don't do anyhting else witht he computer, in the middle of the card.
- clips are red, but it said it has been imported, relink doesn't work, and trying to reimport them give the same result.
-clips become black in the viewer, or there's still grey with the logo, there's only audio or video in the viewer, or in the timeline.
but there are present in the data disk.
Cleaning preferences,closing Fcp, lauching a newlibrary, drag them from the original media doesn't work all the time.
Erasing the clips, and launch a new digit 2,3 times make it works sometimes but it's a lack of time.
-In the timeline, sometimes,the clip becomes black, and after closing and opening fcp x 10.1.3 , it appears .
Do u have any idea why it happens, we have 16gb Ram?

How are you importing the P2 clips?

Are you ingesting straight from the cams using the USB connector; are you ingesting from the P2 cards in a card reader; or are you ingesting from the P2 card structure that you have copied onto your system's hard drive?
