What do you want on the slate?

I'm seeing more use of iPad 'slates' in corporate interviews and voxpops, which should be good, but as an editor, the absolute NUMBER ONE thing I need to know is 'who is this, how's the name spelled, what should the caption say?' - that often gets left out, but I do know the F-stop and the DoP.
And that got me thinking because I am actually an editor who shoots and now shoots more than he edits. I use Movie Slate to get Interviewee name, affiliation, client, location, shot type (interview/voxpop) and so on.
Slightly less importantly, I try and slate 'Ch1: 416 Boomed, Ch2: COS11 Lav'
So, let's say we can slate all interviews and at least some GVs.
Never mind the 'feature film' and indie market with their requirements, what do we - the corporate, short form, 'bunch of random stuff - make it work' editors want on our slates?
I was going to list so many options, but hey - let's just shoot the breeze here. And it's not just slates, it's logging too.
